Identify the processes and assessment tools involved in planning support for individuals with different needs who use social care services.
In this assignment I am going to be identifying the process involved in planning support for individuals with different needs. The two individuals I will be referring to in this assignment are Sarah Devlin and Ruby Hemstock they both have different needs and both use health and social care services.
The process of assessment and planning
Referral-

Implementing the care plan
This stage of the care plan cycle involves the care plan manger to identify dates in consultation with the individual for when services are to be made available.
Sarah- As she will require up to four visits daily, her care plan manger needs to inform her when this will begin. The time and dates.
Ruby- As she will require weekly physiotherapy, her care plan manger also needs to inform her when this will begin for her, the time and dates.
Monitoring
This stage of the care plan cycle involves the care plan manger to discuss with the individual and other significant to their care, about what’s working well and identifies anything which needs changing.
Sarah- As she is suffered another stroke her care plan does need to change to provide support for her individual needs. This stroke has caused a physical and emotional stress to her main carer her husband Chris, which is affecting his health this needs to be addressed. He needs more help to be provided for Sarah to take the strain off him.
Ruby- As her falls are becoming for frequent, her care plan needs to adjust also. She needs to be advised of what could happen if she was to fall again this time badly with no help could lead to her death.
Reviewing
This stage of the care plan cycle involves the care plan manger to review provision of care with individual and other professionals; they would then identify any new goals and targets and alter
